Quest Village (QV):

 

A permanent housing program with supportive services for chronically homeless disable adult men & women. QV is a “Shelter Plus Care” rent subsidized program.

Quest Village provides affordable housing and supportive services to men and women who meet the HUD definition of chronically homeless and who are living with disabilities such as severe mental health diagnosis, physical disabilities, and chronic history of substance/alcohol abuse.     

In order to promote stable independent living, Quest Village provides the following on site services to our residents:

  • Case Management
  • Mental Health/Substance Abuse groups
  • Anger Management Classes
  • Linkage to community resources
  • Job Readiness Classes/GED Classes
  • MARTA Assistance

The following documentation is needed to be considered for a housing opportunity:

  • Documentation of Current MH/SA Diagnosis (Axis I-V)
  • Annual SSI Benefit Award Letter/Proof of Income
  • Signed Disability Verification Form(HUD McKinney Form)
  • Agency Referral Letter (detailing chronically homeless status)
  • 2 Forms of Identification (valid state ID, social security card)
